Summit Policies & Attendee Agreements By registering and attending the 2026 ZERO Prostate Cancer Advocacy Summit, all attendees agree to the liability waiver for any assumption of risk by attending the Summit, as well as a simple media release and compliance with our Standards of Excellence (e.g., professionalism and behavior while representing ZERO and the movement to end prostate cancer). Expand All Liability Waiver Release of Liability and Assumption of RiskThe individual named below (referred to as “I” or “me”) desires to participate in ZERO Prostate Cancer Summit being held in Washington DC (“Conference”) and sponsored by ZERO (the “Company”). In consideration of being permitted by the Company to attend the Conference, the intangible value that I will gain by participating in the Conference and in recognition of the Company’s reliance hereon, I agree to all the terms and conditions set forth in this agreement (this “Release”).I am aware and understand that attending the Conference requires travel to Conference locations and transportation to Conference sessions, including those held at the United States Capitol. The Conference also includes a group dinner where alcohol may be offered, served, and/or consumed. Conference participants will stay for one or more nights in a hotel and will be provided meals throughout the Conference. The activities listed above are referred to as (“Conference Activities”) Attending the Conference involves the risk of personal or psychological injury, pain, suffering, temporary or permanent disability, death, property damage, and/or financial loss. I am also aware of the highly contagious nature of bacterial and viral diseases, including COVID-19 (collectively, “Disease”) and the risk that I may be exposed to or contract the Disease by attending the Conference and engaging in the Conference Activities, which may result in illness, personal or psychological injury, pain, suffering, temporary or permanent disability, death, property damage, and/or financial loss. I acknowledge that these risks may result from or be compounded by the actions, omissions, or negligence of Company employees or others, including negligent emergency response or rescue operations of the Company. I understand that while the Company has implemented measures to reduce the risk of injury at the Conference and the spread of the Disease, the Company cannot guarantee that I will not be injured or become infected with the Disease while attending the Conference or during my participation in the Conference Activities and that Attending the Conference may increase my risk of contracting the Disease.NOTWITHSTANDING THESE RISKS, I ACKNOWLEDGE THAT I AM VOLUNTARILY ATTENDING THE CONFERENCE AND PARTICIPATING IN CONFERENCE ACTIVITIES WITH KNOWLEDGE OF THE DANGERS INVOLVED. I covenant not to make or bring any such claim against the Company or any other Releasee, and forever release and discharge the Company and all other Releasees from liability under such claims.I confirm that I am: (a) in good health and proper physical condition and do not have any medical or other conditions that would impair my ability to participate in the Conference or Conference Activities; and (b) not experiencing symptoms of the Disease, such as cough, shortness of breath, sore throat, congestion, headache, muscle or body aches, chills, or fever. If at any time I believe conditions to be unsafe, that I am no longer in proper physical condition to attend the Conference or participate in Conference Activities, or I begin experiencing symptoms of the Disease, I will immediately discontinue attending the Conference or participating in Conference Activities and contact summit@zerocancer.org immediately. I agree to comply with Company’s stated instructions, recommendations, cautions and terms and conditions for attending the Conference or participating in Conference Activities. I will comply with all federal, state, and local laws, orders, directives, and guidelines related to the attending the Conference or participating in Conference Activities. I acknowledge that the Company is relying on these statements to allow me to condition to attend the Conference or participate in Conference Activities.This Release constitutes the sole and entire agreement of the Company and me with respect to the subject matter contained herein and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter. If any term or provision of this Release is invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Release or invalidate or render unenforceable such term or provision in any other jurisdiction. This commitment also means providing the ZERO community with a harmonious, respectful, diverse, and cooperative environment. To add clarity to ZERO’s expectations, the Standards of Excellence Committee and the Standards of Excellence Policy were created.
